Lipedema

If you’re reading this, you probably know that lipedema is not just ordinary fat. As a result, living with lipedema can be a challenging journey, marked by physical discomfort, emotional distress, and the frustration of a condition often misunderstood or misdiagnosed. If you’re searching for “lipedema surgery near me,” you’re taking an essential step toward improving your health, comfort, and quality of life. Dr. Sepideh Saber can.

Lipedema liposuction can remove the painful, disproportionate fat deposits that accumulate in the legs and lower extremities. Lipedema liposuction targets the diseased fat tissue that causes swelling, tenderness, and a heavy, aching sensation in the thighs, calves, and ankles, restoring both comfort and proportion to the lower body.

What is lipedema?

Lipedema is a chronic and progressive medical condition. It is characterized by a symmetrical buildup of fat. It usually affects the lower body. In other words, the hips, thighs, lower legs, and sometimes the arms. It is not just obesity (excess fat), and it does not improve with a traditional diet or exercise.

The disproportionate buildup of painful fat and diseased fatty tissue occurs almost exclusively in women. Over time, lipedema may worsen, leading to increased pain, mobility limitations, and emotional distress. A successful lipedema treatment plan requires care from a knowledgeable and board-certified plastic surgeon who can tailor a comprehensive treatment plan best suited to your needs

Who does lipedema usually affect?

Lipedema affects about 11% of women worldwide. Although cases in men have been reported, this is rare. The main reason for this inequality is that hormonal changes during puberty, pregnancy, or menopause often trigger the start or progression of lipedema. Genetic factors are also a cause.

What are the telltale signs and symptoms of lipedema?

Because lipedema is not merely ordinary adipose tissue (body fat), recognizing its signs can facilitate early diagnosis. Here are the main things to look for:

  • Disproportionate fat distribution: Characterized by bilateral (on both sides), symmetrical swelling of the legs (upper and lower legs), hips, and sometimes arms. The hands and feet are typically unaffected, creating a distinct “cuff” at the ankles or wrists. The upper body remains comparatively slim.
  • Pain and tenderness: Affected areas are often painful, tender, or heavy.
  • Easy bruising: Many people with lipedema find they bruise very easily in the affected limbs, even without significant trauma such as a fall or collision with an object.
  • Resistance to weight loss: Lipedema fatty tissue does not respond to strict dieting or exercise.
  • Psychological impact: The physical changes and chronic pain can lead to significant emotional and psychological distress, including anxiety and depression.

What are the different stages of lipedema?

Lipedema typically progresses through four stages, each with distinct physical attributes. Of course, Dr. Saber will identify the stage to guide your personalized treatment plan.

  • Stage 1: The skin is smooth, but the underlying fat tissue has started to enlarge. Swelling may increase during the day.
  • Stage 2: The skin develops indentations and a texture similar to a mattress, and larger fat nodules can be felt.
  • Stage 3: Large masses of fat and skin cause significant lobules (skin flaps) and deformities, particularly around the thighs and knees.
  • Stage 4: Lipedema is accompanied by lymphedema (lipo-lymphedema), in which the lymphatic system is compromised, leading to additional fluid buildup.

This link to the Lipedema Foundation website has excellent pictures that clearly show the different stages.

What is the difference between lipedema and obesity or lymphedema?

Lipedema is frequently misdiagnosed as simple obesity or lymphedema. However, there are key differences:

  • Lipedema vs. obesity: Lipedema fat is painful, disproportionate, and resistant to diet and exercise. Obesity is generalized and often responds to lifestyle modification.
  • Lipedema vs. lymphedema: Lipedema involves abnormal fat deposition and is typically symmetrical. Lymphedema is an accumulation of lymphatic fluid that often affects only one limb and is characterized by swelling in the hands and feet.

Dr. Saber knows that an accurate diagnosis is essential.

How does Dr. Saber diagnose lipedema?

Dr. Saber’s diagnosis depends on a thorough medical history and physical exam. This includes an assessment of fat distribution, tissue texture, pain, and the sparing of the hands and feet. There is no single blood test or imaging scan. However, experienced lipedema specialists can reliably diagnose lipedema based on clinical evaluation.

What are the comprehensive lipedema treatment options?

There are both non-surgical and surgical options. While there is no cure for lipedema, treatment can significantly reduce pain, improve mobility, and enhance quality of life.

Conservative (non-surgical) management options include:

  • Compression therapy: Wearing specialized compression garments is crucial. One study found that most patients reported their symptoms improved with the use of compression garments. Compression reduces swelling, supports tissues, improves blood flow, and promotes the movement of lymphatic fluid.
  • Manual lymphatic drainage (MLD): A gentle massage technique performed by a certified therapist to stimulate the lymphatic system and reduce fluid buildup.
  • Diet and nutrition: While diet cannot eliminate lipedema, adopting an anti-inflammatory diet may help manage inflammation and support overall health. Consulting with a qualified nutritionist is recommended.
  • Low-impact exercise: Activities such as swimming, water aerobics, and cycling can improve mobility and circulation without stressing painful joints.

Surgical options: the gold standard for lipedema fat removal

For patients with lipedema who experience persistent pain and functional limitations, lipedema liposuction is the most effective treatment. It is not a weight-loss procedure. It is performed to remove diseased tissue, reduce pain, and improve function.

The goal of lipedema surgery is to remove abnormal fat while preserving lymphatic vessels.

Depending on the extent and anatomy of the disease, treatment may be performed in a single surgical setting or staged across multiple procedures to optimize safety and outcomes.

  • Tumescent Liposuction: This is the foundation of modern lipedema surgery. A large volume of tumescent solution—a mixture of saline, local anesthetic (lidocaine), and epinephrine—is injected into the fat. This causes the fat cells to swell and become firm, numbs the area, and constricts blood vessels to minimize bleeding. This makes fat removal safer and more efficient.
  • Power-Assisted Liposuction (PAL): This technique uses a specialized cannula that vibrates gently. This mild vibration facilitates the dislodgement of fat cells, enabling effective fat removal with less trauma to surrounding tissues, including the crucial lymphatic vessels. This can lead to a quicker recuperation period.

What are the common surgical procedures used in lipedema treatment?

Dr. Saber offers many common surgical procedures, and your surgical care will be tailored to your individual situationbased on disease distribution, the type and severity of your symptoms, and your functional goals. The goal is to enhance your comfort, mobility, and quality of life through controlled debulking combined with intentional shaping and contouring—not just simple fat removal—while safeguarding your lymphatic structures.

The common lipedema surgery procedures include:

Lipedema liposuction (legs and lower extremities)

Our patients who undergo lipedema liposuction of the legs and lower extremities often report significant reductions in pain, improved mobility, and a dramatically better quality of life, finally finding relief from a condition that can be deeply isolating and physically debilitating.

Using advanced techniques, Dr. Saber carefully extracts the fat while preserving the lymphatic vessels, reducing the risk of further lymphatic damage and alleviating the progressive symptoms associated with the condition.

Lipedema liposuction (arms and upper extremities)

Lipedema affecting the arms and upper extremities can cause disproportionate, painful swelling that resists diet and exercise, leaving many women feeling frustrated and self-conscious despite their best efforts.

Dr. Saber’s specialized lipedema liposuction for the arms targets the abnormal fat deposits that accumulate in the upper arms, forearms, and surrounding tissue, using gentle tumescent techniques designed to remove diseased fat while preserving lymphatic vessels and minimizing recovery time.

Unlike traditional cosmetic liposuction, this procedure is performed with a therapeutic goal: reducing pain, heaviness, and restricted mobility that lipedema patients experience daily. The result is not only a more proportionate silhouette but also meaningful relief from the chronic discomfort and tenderness that define this condition, helping patients reclaim confidence and comfort in their own bodies.

Abdominal lipedema surgery, lipedema tummy tuck, and abdominoplasty

In select patients with abdominal involvement, treatment may include abdominal liposuction with or without excision (surgical removal)—sometimes referred to as a lipedema tummy tuck or abdominoplasty—to address discomfort, postural strain, mobility limitation, or difficulty with compression garment fit.

The focus is on functional improvement and body contourbalance, not just on cosmetic enhancement.

Thigh lift for lipedema

Removal of redundant thigh skin when laxity (looseness) interferes with mobility, hygiene, or effective compression.

Breast reduction and breast lift (mastopexy) in lipedema patients

For women living with lipedema, abnormal fat accumulation often extends to the chest and breast tissue, leading to disproportionate breast size that compounds the physical and emotional burden of the condition.

Breast reduction combined with a breast lift (mastopexy) offers meaningful relief by removing excess breast tissue and skin, repositioning the nipple and areola to a more natural height, and restoring a proportionate, lifted contour.

For lipedema patients specifically, this procedure can alleviate chronic neck, back, and shoulder pain caused by the added weight, improve posture, and reduce skin irritation beneath the breasts that is common when lymphatic function is compromised.

Beyond the physical benefits, many patients report a profound improvement in body confidence and quality of life, finally feeling as though their silhouette reflects who they are rather than the condition that has defined their body for years.

Combined or staged procedures

Procedures may be combined or staged to optimize safety, lymphatic preservation, and recovery.

People with lipedema often search for terms such as lipedema leg surgery, lipedema arm surgery, lipedema tummy tuck, or abdominoplasty. While some or all of these may be relevant to your situation, please understand that the appropriate approach will be determined during consultation with Dr. Saber and will be tailored to your individual anatomy and functional goals.

When should your surgeon enhance results with excision?

The short answer is when it is medically indicated. Let’s dive in a little deeper.

Following lipedema liposuction or when the disease is more advanced, some patients may develop excess or redundant skin that interferes with mobility, hygiene, or effective use of compression garments.

In these cases, surgical excision—such as a thigh lift, an arm lift, or abdominal excision (including abdominoplasty when appropriate)—may be recommended to remove excess tissue and improve functional comfort, mobility, and garment fit. The decision to pursue excisional surgery is individualized and based on your anatomy, type and number of symptoms, and overall treatment goals

Why does early lipedema diagnosis and treatment matter?

Early diagnosis and appropriate treatment may help slow disease progression, reduce pain, preserve mobility, and lower the risk of lipo-lymphedema. This helps to increase long-term quality of life.

Is lipedema surgery covered by insurance? Is there financing?

Insurance coverage varies by plan. Insurers may require a formal diagnosis, documentation of failed conservative therapy, and a letter of medical necessity. Financing options and medical spending accounts can be discussed during your consultation.

What does lipedema surgery recovery look like?

Recovery timelines vary, but most patients experience gradual improvement over weeks to months when postoperative care and the recommended treatment plan are adhered to.

Post-op lipedema care may include:

  • Compression garments
  • Pain management
  • Leg elevation
  • Light activity to promote circulation
  • Manual lymphatic drainage (MLD) with a certified therapist when appropriate
  • Scheduled follow-up visits

I was fortunate to find Dr. Saber after researching board-certified surgeons who specialize in breast cancer reconstruction revision surgery. I knew I was in good hands from the moment I spoke with the lovely and efficient Marlene, Dr Saber's office manager. Dr Saber patiently listened to my concerns and, after examining me, explained what I could realistically expect the outcome to be from another surgery. She also explained to me that she would need to review my previous surgery notes before we could discuss a possible further surgery, which I found comforting and highly ethical. I also discussed a couple of other procedures that I would like done, including fat removal from my large upper arms that had caused me great embarrassment for several years. Dr. Saber is an excellent surgeon who is incredibly kind and genuine. She truly understands the female body and the emotional aspect of body insecurities yet, she is transparent about what can and cannot be done and takes time to discuss realistic expectations and outcomes. My breast reconstruction revision surgery will take place in the New Year, but in the meantime, I have the most fabulous upper arms that I recently flaunted in a strapless dress (something I have not done in years). Dr Saber is truly one of a kind, and I still cannot believe how fortunate I am to have found her. I highly recommend Dr Saber.

I don’t have enough wonderful words to describe Dr Saber and Marlene! I had a tummy tuck done this year and from start to finish the entire experience was amazing. Marlene is always so helpful and works so hard to make every detail a breeze for you. When I had my consultation with Dr Saber I was at a point of complete agony with my body. She instantly took that stress and frustration off my shoulders! As a woman not being heard by doctors for years I left that consultation with happy tears knowing I had finally found my Dr. It was a major surgery and I felt completely zen and at peace going in because she really does make you feel that safe. I didn’t think I would ever look the way I do now! She gave me the gift of having the body I kept fighting for. I will never stop recommending her.

I saw Dr. Saber when I broke my wrist. I ended up having to have surgery. She did an excellent job and I am healing nicely. She was very professional, but was also very kind and compassionate. I would highly recommend her!!

I LOVED DR SABER!!! There are a few reasons why I chose her. One was because she’s a female Doctor that was my goal who also did a good job (which I was able to confirm with all her before and after photos) and what sold me was my safety in mind. She made me feel like I was in the best hands ever along with Marlene who coordinated everything THANK YOU LADIES! Dr Saber helped me get back the body I have always desired. I had a lot of stomach fat from weight gain and I lost weight so a lot of my weight was in my front lower and upper abdomen. Now I have a super beautiful curvy body like an hourglass after a tummy tuck and lipo she did. She understood the assignment hahaha . I’m 32 years old no kids just wanted to enjoy my younger body looking and feeling beautiful and she really took good care of me. Even after hours I had an emergency due to my fault but she was there and got back to me very quickly and I’m very grateful. I also think her work says a lot because I stopped taking pain medication on the 3rd day after surgery and was fine with very minimal pain I was able to tolerate the healing process. Highly recommend going with her I just had a wonderful experience and would do it all over again if I had too. Maybe after I have kids. Thanks Dr Saber and team <3
